The necessity of palliative care for children
2. Personalized care
The department provides hospital care, outpatient care and home care, or centralized care if the child and family wish. The addition of palliative care and services for patients with complex chronic diseases does not necessarily mean that the child needs to change the treatment plan (the child will continue to be seen by a specialist). For children in home care, the staff will provide guidance through the analysis of the information obtained by the children, so as to avoid the inconvenience caused by travelling to and from the hospital and the interruption of daily life caused by long-term hospitalization.
3. Family care training
An important function of the department is to train family caregivers so that they can care for children at home. These children's situations are complex, and professional training enables caregivers to take effective action in different situations. After the child goes home, the service does not stop, and family caregivers can always access counseling services from here. In addition to training and support in illness care, the department also helps families deal with sick children and provides them with immediate emotional support and psychological counseling.
4. Multi-department collaboration
The Palliative Care and Complex Chronic Disease Patient Services Department works closely with other departments at SJD Barcelona Children's Hospital. All staff involved with the child work together as a team to develop a treatment plan that includes treatment goals, the child's family situation, possible problems, and response options. The department also links with primary care as well as other teams, such as those in PADES (Home Care Planning and Support Team). In addition, other systems other than health care (social services, education system......) Establish communication.
